General Critical Care Teaching and Training

WELCOME!

We hope you enjoy your time in ICM. You have a diversity of educational needs, and we hope you can get the training you want between the rich clinical case mix and formal teaching here. There are plenty of opportunities for clinical learning, procedures and supervised patient management. Below is a list of meetings and activities you can get involved in while you are here and our educational time agreement. Our biggest ask is that you practice and promote a culture of looking after each other in the team. It is a spread-out critical care service so check that colleagues aren’t swamped and share out the load if you can.

For new nurses and specific nurse teaching including STEP competencies there is a separate section under construction.

EDUCATIONAL SUPERVSION

You will be allocated an ES +/- clinical supervisor. Contact the secretaries if you don’t know who you have been allocated. You should make an appointment with them in the first two weeks. Prior to the meeting, think about areas you would like to improve and projects you would like to explore. Your supervisor will discuss and agree an educational contract with you including how you will use your no-clinical time. You will then have a mid-point and end of attachment meeting (with additional meetings if required). Responsibility for arranging meetings and completion of your portfolio is your own. 

INDUCTION RESOURCES

Please view the central line insertion and other packages on your email or on your teams' files in preparation for clinical work and procedure training. 

AGREED EDUCATIONAL TIME EDT / SDT

1)    All colleagues have non-clinical paid hours. These are for departmental teaching/SPA/portfolio and can be delivered flexibly

2)    You should agree with your ES what you will use this time for and will be expected to demonstrate your activities

3)    Failure to attend departmental teaching and demonstrate SPA activities could prevent satisfactory completion of this post 

4)    For external SL, submit requests to ICU rota maker at least 6 weeks in advance. Where rota-makers are unable to grant SL a lieu day at a mutually convenient time may be considered when you have attended in your own time. Please ask

5)    You may ask the day team if you can attend your own choice of educational activity at the following times: 

      • When allocated to an Outreach session
      • When allocated to a non-clinical session 
      • On occasions where staffing and work load allow, the team agree to cover your duties and you are available to return if required
      • Mandatory specialty local teaching programmes (i.e IMT teaching) ONLY IF ICU TEACHING IS NOT ON. If you are on-duty you should attend ICU Sim and teaching.
6)   Activities in trainees own time are not expected but offered- we will maximise usefulness i.e exam practice, interview practice, dialling in to sessions etc 

THURSDAY DEPARTMENTAL TEACHING AND MDT SIMULATION

These are built into your rota each Thursday.

MDT In-situ Simulation takes place from 13:30-14:30 in GCC (or in GCC seminar room if no side rooms). You will participate or observe. Senior trainees may be asked to help deliver.

Tutorial style teaching takes place from 14:30-16:30 in GCC seminar room and via ms teams . The timetable rotates between Junior and middle grade teaching. You are most welcome to attend either session, but you must ensure the intended recipients are freed up from clinical work to attend. We expect you to attend or dial in to 60% of teaching. Your attendance is registered by completing the feedback link (you need your NHS mail portal login). 

Please don’t forget to do this small curtesy to your speakers!

Please note, we do not expect you to attend when you are on the rota  for night shift. Discuss with your ES if you are finding difficulty attending either on-duty or remotely. The links for videos of many sessions are available on the MS teams files.

DEPARTMENTAL TEACHING TIMETABLE

JOURNAL CLUB    

I-line (Dr Rob Darke and Ben Beatty) are hosting regional ICU journal club. You email vjc@a-line.org.uk to be added to the ‘team’ in order to attend and to request to present. 

CLINICAL GOVERNANCE MEETING

You can attend for education or to present audits, QI projects or new guidelines. Please contact Dr Isabel Gonzalez

They occur on 2nd  Wednesday of the month 12:00- 13:30.

Dates are available from the Secretarial Team.


MORBIDITY AND MORTALITY REVIEW

You are expected to prepare at least one review summary while you are here.

 You will discuss it with your ES or CS who will submit the review to the hospital system. If you want to present it to the department, please discuss you’re your ES and ask secretaries who is leading the next meeting.

RESEARCH 

There are opportunities to understand and participate in ICU research. Please speak to Research nurses or Dr J Henning.

Completing your GCP qualification is your first step (4 hours e-learning). The research team have monthly meetings. Speak to one of the team for dates.


CRITICAL CARE MULTIDISCIPLINARY REVIEWS

The general critical care MDT meets  every Tuesday 13:00-14:00 in GCC seminar room for longer term patients (ask OT or senior physiotherapists).

Opportunity to present or to map to MDT ICM competencies.

Trainees SHADOWING Registrars or CCOT 

This should be agreed between team members where adequate cover on units can be ensured.


MANAGEMENT EXPERIENCES FOR MIDDLEGRADES

1)    Daily handovers ICU seminar room 
2)    Senior staff meetings (monthly- check with Secretaries)
3)    Medical Examiner session (contact Judith Wright)
4)    Advanced trainees: Insights from the CD’s perspective (contact Michelle Carey) 
5)    Contribute to departmental education: help run simulation, nurses airway assistants course, nurse and trainee doctor induction programme, training for transfer and sub-specialty days
6)    Contribute to “Generic skills for FY2 Doctors” (ask Isabel Gonzalez)

POCUS/ECHO

We have a range of high-quality machines for echo and POCUS. We have supervisors and mentors in the department to support you through POCUS accreditations (contact Dr A Hyde).

Mentors: AS, AH, SW, EW, DMc, MS, UF, JM, AP, AW to name a few. Dr Andy Hyde is running additional Echo teaching events. Contact him to be included.


            CLINICS / THEATRE / OTHER MEETINGS

    • North East Assisted Ventilation Service (home ventilation): Twice per month clinic on Fridays. Run in JCUH by the RVI home vent consultants. Arrange with Dr Isabel Gonzalez.
    • ICU follow-up clinic. Please contact Dr D Monkhouse if you would like to attend. Here is a great video in the meantime. 

INTENSIVE CARE UNIT CATALYST FILM

    • Bronchoscopy clinic. You are most welcome to observe the Respiratory Physicians. Best time is Friday pm. They do 5 or 6 standard Bronchoscopies. Plan to observe the procedure, tricks and anatomy. I wouldn’t expect to get hands-on but you might be lucky. Email the respiratory secretaries (Victoria Ashmore or Lesley Adams) to ask to attend and find out who is the consultant that week.
    • Other specialty or taster clinics: If you wish to arrange to attend for a taster or skill refresher we will support if we can. Please see EDT agreement. 
    • Theatre taster or skill refresh sessions: Limited availability. Requests should be directed through Dr Andy Hyde
    • Trauma MDT meeting: every few months: O and T, ED and ICU. Ask your ES or Dr Laura Evans (ED) for the dates and link.
